JOB SUMMARY
-
Under the direction of the Registered Nurse, provides total patient care in accordance with physician’s orders and individual patient needs in conformance with the recognized standards of nursing practice.

DUTIES AND ESSENTIAL FUNCTIONS
- Assesses patient and/or conducts screenings and documents utilizing standard scales when indicated as required per organization.
- Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family, and health care team.
- Documents changes in patient condition and effectiveness and timeliness of care/intervention.
- Utilizes organization-wide safe medication practices including, but not limited to the 5 rights of medication administration, barcode scanning and other medication safety processes specific to the unit/department.
- Knows actions and usual dosage, indications and side effects of medications administered.
- Provides teaching and educational material to the patient and/or significant other as needed according to the care, treatment services and identified risks specific to the patient and documents patient and/or family understanding.

EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS

Minimum requirement:
-
Graduate from a Practical Nursing Program.
-
Maintains active LPN license in the Commonwealth of Kentucky and any other states as applicable.

Achieves and maintains American Heart Association Basic Life Support (BLS).
