JOB SUMMARY

Assists patients and their families with personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefits from medical care, by performing the following duties.

DUTIES AND ESSENTIAL FUNCTIONS
- Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient’s health problem.

Overall performance rating of below indicated essential job duties:
- Helps patients and families through individual or group sessions to understand, accept and follow medical recommendations.
- Develops, evaluates, revises and implements psychiatric evaluations and educational materials.
- Prepares psychosocial assessments with diagnosis and treatment recommendations, follow-up plans and documents in the medical record as needed.
- Provides follow-up care to restore patient to optimum social and health adjustment within patient’s capacity.

EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS

Minimum requirement:
-
Master’s degree (M.S.W.) in Social Work
-
Two years psychiatric patient experience
-
Licensed Clinical Social Worker (L.C.S.W.) in the State of Kentucky

Works indoors in an office/clinic setting
