Job Description:
Job Summary
Responsible for providing interactive clinical communication with callers to deliver personalized health management assistance. Utilizes expert interviewing skills to assess, educate, and guide pediatric and adult patients during and after physician office hours. Demonstrates empathy while offering information, education, and guidance through the nurse triage/access center. Coordinates with healthcare providers to ensure continuity of care. Maintains accurate documentation of all interactions to support patient safety and quality care.
Essential Functions
• Answers the telephone triage line promptly and courteously.
• Secures and manages electronic medical records for patients requesting appointments, prescriptions, and consultations.
• Advises callers, directs them to the appropriate level of care, and selects triage plans using established protocols, documenting actions in EPIC.
• Collaborates with physicians, urgent care staff, office staff, and referral centers to ensure accurate information and efficient processes.
• Maintains current knowledge of services, resources, diseases, procedures, diagnostics, treatments, and medications.
• Performs timely follow-up calls after initial contact to ensure continuity of care.
• Participates in data collection and interprets information to support process improvement.
• Performs other duties as assigned.
Education Requirement
Associate’s degree in nursing
Bachelor’s degree in nursing (BSN) is preferred
Experience Requirement
3-5 years of experience in nursing or other relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
Certifications & Licensures
Current and active Registered Nurse (RN) license in applicable states, Basic Life Support (BLS)
