JOB SUMMARY
-
The Enterstomal Therapy nurse provides a comprehensive ostomy, skin, and wound care program for inpatients at KDMC. The ET nurses act as a consultant to access, plan, implement, evaluate, and reassess the care of patients with abdominal stomas, acute/chronic wounds, and incontinence. The ET nurse provides services to patients ranging from adult through the geriatric developmental life span.

DUTIES AND ESSENTIAL FUNCTIONS

Effectively assesses and reassesses patients based on criteria that are both age and culturally appropriate for the patient; Assesses/reassesses patient needs using objective and subjective assessment of the physical, psychological, emotional, spiritual, cultural, and environmental needs of the patient.

Identifies appropriate patient needs and nursing diagnoses from the collected assessment data and plans care appropriately; Formulates and documents a plan of care based on assessment and in collaboration with the patient, family, and/or significant other, as well as appropriate members of the multidisciplinary patient care team.

Updates and documents the plan of care daily and as indicated based on ongoing assessment; Administers medication correctly/safely utilizing five rights, two patient identifiers and double checks of high risk medications per policy.

Performs nursing procedures, interventions and actions safely according to hospital policy/procedures; Consistently and thoroughly completes documentation on correct forms per medical center/unit policies.

Coordinates and provides discharge planning and patient and family education throughout hospitalization; Provides care that is age appropriate and culturally sensitive; Evaluates the appropriateness of individual patient’s response to the nursing care and modifies the plan as evidenced by chart documentation.

Recognizes change in patient’s physical and mental status and informs physician and/or other health care professionals as evidenced by chart documentation.

Participates in Performance Improvement initiatives by evaluating outcomes of patient care and participating in data collection.

EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS

Minimum requirement:
-
Graduate of a school of nursing and maintains current licensure as a Registered Nurse in the Commonwealth of Kentucky or other NLC state with multistate privileges.

Achieves and maintains current American Heart Association Basic Life Support (BLS) Healthcare Provider Certification.
