Job Description:
Job Summary
Responsible for managing cardiovascular care for outpatient populations in collaboration with multidisciplinary teams across multiple clinic locations. Coordinates patient care in ambulatory settings, partners with cardiology testing groups, and facilitates communication between outpatient and inpatient teams. Delivers evidence-based interventions, ensures clinical outcomes, and maintains timely documentation. Demonstrates advanced clinical knowledge, technical competency, and professionalism in all interactions. Pursues ongoing professional development to support optimal patient care.
Essential Functions
• Manages cardiovascular care for patients in the ambulatory clinic setting.
• Collaborates with multidisciplinary healthcare teams to coordinate patient care.
• Oversees outpatient management and serves as a resource for inpatient consultative care.
• Coordinates with outpatient cardiology testing groups such as ECHO and heart station.
• Applies evidence-based practice and advanced clinical knowledge in patient care.
• Educates, mentors, and precepts students, nursing staff, and novice APPs.
• Participates in research, quality improvement, and evidence-based practice initiatives.
• Engages in professional development, maintaining licensure, CEUs, and accreditation requirements.
• Maintains a positive, professional demeanor and fosters effective communication with patients, staff, and the community.
• Performs other duties as assigned.
Education Requirement
Master’s degree in a nurse practitioner program, physician assistant program, or a related field
Experience Requirement
0+ years of experience in a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
