Job Summary

Responsible for collaborating with multidisciplinary teams to deliver comprehensive primary and behavioral health care across ambulatory and inpatient settings. Conducts examinations, writes prescriptions, and develops thorough health care plans for diverse patient populations. Applies evidence-based practice, ensures safe, care, and demonstrates autonomy in medical decision-making. Maintains professionalism, effective communication, timely documentation, and ongoing professional development. Coordinates education, research, administrative services, and adapts to varying patient volumes and care environments.


Essential Functions

• Assesses patient status through interviews, chart reviews, and physical examinations.

• Orders and interprets diagnostic tests and screenings.

• Develops and implements treatment plans, including prescribing medications and procedures.

• Educates patients and families on health promotion, illness prevention, and treatment plans.

• Coordinates care and communicates case management information with clinical teams and community resources.

• Documents clinical activities promptly and accurately in medical records.

• Collaborates with multidisciplinary teams to manage acute and chronic medical needs.

• Participates in quality improvement, education, and professional development activities.

• Maintains professional licensure, CEUs, and compliance with accreditation requirements.

• Performs other duties as assigned.


Education Requirement

Master’s degree in a nurse practitioner program, physician assistant program, or a related field



Experience Requirement

0+ years of experience in a relevant area


An equivalent combination of education and experience may be considered.

All experience must be paid and in the same related field.

Part-time and PRN experience will be prorated based on hours worked per week.

Volunteer work and internships for academic credit are not counted.


Certifications & Licensures

Current and active Registered Nurse (RN) license, Advanced Practice Registered Nurse (APRN), Family Nurse Practitioner (FNP) or Physician Assistant-Certified (PA-C), Basic Life Support (BLS)
