Job Description:

Job Summary

Responsible for conducting physical therapy evaluations and administering treatments in accordance with physician orders and regulatory standards. Provides direct patient care for individuals with diverse diagnoses to restore function, relieve pain, and prevent disability. Performs thorough documentation of assessments, interventions, and outcomes in the medical record. Collaborates with healthcare teams to ensure quality care and supports program development initiatives. Adheres to organizational policies and procedures to maintain compliance and enhance patient recovery.


Essential Functions

• Assesses and redistributes resources to enhance Rehab Services.

• Participates in projects and activities to improve departmental operations.

• Bills appropriately and meets departmental productivity standards.

• Performs patient evaluations and delivers treatment for various diagnoses.

• Completes chart reviews and documents all aspects of patient care in the medical record.

• Collaborates with healthcare professionals to coordinate and discuss patient care plans.

• Educates patients and caregivers regarding care, restrictions, and precautions.

• Delegates responsibilities to PTAs and supervises students as appropriate.

• Provides or coordinates orientation, training, and education for staff and students.

• Performs other duties as assigned.

Experience Requirement

0-2 years of experience in physical therapy, rehabilitation, or patient care, or a relevant area


An equivalent combination of education and experience may be considered.

All experience must be paid and in the same related field.

Part-time and PRN experience will be prorated based on hours worked per week.

Volunteer work and internships for academic credit are not counted.


Certifications & Licensures

Maintains active licensure as a Physical Therapist. Achieves and maintains American Heart Association Basic Life Support (BLS)
