JOB SUMMARY
-
The primary purpose of this position is to provide management of resident care as related to the Resident Assessment Instrument (RAI) process, in accordance with federal and state regulations, and as directed by the Director of Nursing and Administrator. The MDS Coordinator/MDS Nurse is expected to have access to and follow the most recent version of the RAI User's Manual at all times.
The MDS Coordinator/MDS Nurse is accountable for helping achieve the organization's goals for positive outcomes. This is to be achieved through effective processes, monitoring, training and implementing systems that will lead to appropriated MDS completion and submission, care planning, billing practices, utilization of resident's benefits and accurate reporting of quality measure data.
The position serves as a resource to all staff regarding documentation requirements for resident MDS's and Care Plans. Ensures that all assessments and records pertaining to the Resident are current and descriptive of the care rendered. Evaluates progress of each patient with clinical oversight and management.

DUTIES AND ESSENTIAL FUNCTIONS

Overall performance rating of below indicated essential job duties:

Conduct initially and periodically a comprehensive, accurate standardized, reproducible assessment of each resident upon admission, annually (not less frequently than every 366 days) and whenever a significant change as identified in the resident’s status as defined in Chapter 2of the RAI User’s Manual.

Assess every resident using the quarterly item set not less frequently than once every 92 days.

Conduct PPS assessments, including timely opening and completion, as outlined in Chapters 2 and 6 of the RAI User’s Manual.

Ensure clinically accurate coding and completion of the MDS assessments and care plan conferences.

Provide oversight of the assessment completion and transmission to the national repository.

Review final validation reports and corrections or modifications in response to warnings or errors as needed.
-
Oversee the baseline care plan and coordinate the completion of the comprehensive care plan according to regulatory requirements.
-
Ensure the Baseline Care Plan Summary is provided to the resident and/or their representative by the completion of the comprehensive care plan.

Assign and coordinate the work of health professionals toward accurate and timely completion of MDS assessments and care plans.

Communicate all MDS schedules and schedule changes to the IDT.

Evaluate progress of each patient to identify need for Interim Payment Assessment. (IPA)

Be highly involved in determining skilled level of care for Medicare residents and procuring required Medicare-specific documentation; be responsible for physician certification of skilled level of care throughout the Medicare stay; be involved in making eligibility determinations.

Coordinate care with case managers for residents utilizing managed care, health maintenance organizations (HMO’s), commercial insurance and other alternate payment models.

Ensure compliance with state-specific regulations regarding the RAI process.

Obtain Quality Measure, SNF SRP and Five Star reports at least monthly to review with the clinical team. Provide insight and analysis of MDS-based Quality Measures (Work with Quality Manager).

Serve as a member of the quality assessment and assurance (QAA) and/or the quality assurance and performance improvement (QAPI) committees.

Work closely with hospital discharge planners and physicians to obtain accurate and complete documentation to support ICD-10-CM diagnosis coding and surgical procedures.

Audit and improve staff education/competency as needed to ensure accurate and timely completion of supporting documentation and MDS assessments.

Participate in care conferences, discharge planning, training, caregiver training and the provision of resources as needed.

Foster effective working relationships and build consensus.

Maintain contact with residents and families, inviting them to attend care plan conferences.
-
Maintain confidentiality of sensitive information. Plan, organize, prioritize, work independently and meet deadlines.
-
Use judgement and make sound independent decisions.
-
Work effectively with individuals at all levels of the organization, as well as with residents, family members, visitors, government personnel and the public.

Be knowledgeable of regulations, practice standards and procedures, as well as laws, regulations and guidelines pertaining to the RAI process.

Possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for ensuring the accurate and timely completion of the RAI documents.

Have the ability to learn computer systems, system applications and related office equipment.

Ensure that the resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are always well-established and maintained.

Provides and/or coordinates the collection of documents and materials requires for the Assessments and Care Plan conference.

Advises the Director on problems associated with the proper and timely completion of MDS’s and Care Plans.

EDUCATION/LICENSES/CERTIFICATIONS/OTHER REQUIREMENTS

Minimum requirement:
-
Minimum graduate of an accredited school of nursing.
-
Current and active RN license as a nurse in the State of Kentucky.

Preferred qualifications:
-
Completion of UKKD LEAD Academy.
-
One year experience providing direct care to residents in long-term care setting.
