Community Services Specialist

  • JOB SUMMARY
    • Organizes and executes community programs that furthers KDHS’s Mission and Vision of measurably improving the health of the community. Serves in planning, developing, and teaching community health and wellness classes, screenings and other related activities. 
  • DUTIES AND ESSENTIAL FUNCTIONS 
    • Implements activities that support health system’s CHNA/CHIP and assures goals are being addressed and met.
    • Overall performance rating of below indicated essential job duties:
    • Trains volunteers on educational and/or health screening equipment and completes competency validations for each volunteer. Assures that all volunteers are trained and validated for assigned duties before event when possible.
    • Serves as leader at events. Organizes and coordinates events and activities both on and off campus as scheduled.  Assist with community health screenings.
    • Assists with scheduling and executing Mobile Health Unit screenings and activities, including Healthy Hearts, EKG’s. Works with the drivers to ensure the units and compliance with KD policies and procedures and Joint Commission standards.
    • Excels at speaking in front of small and large groups and is comfortable working with students and adults individually or simultaneously.  Teaches in schools, churches, businesses and other community based organizations regarding wellness activities including nutrition, exercise, hand hygiene, substance abuse, tobacco and prevention and cessation.
    • Works with the Community Services Specialists regarding scheduling and assignment of upcoming events.  Works with the entire Community team to ensure activities are organized and coordinated. Maintains teaching curriculum and hands-on educational exhibits. Makes recommendations for new purchases
    • Assists with the CPR Training Center and Meals on Wheels as requested.
    • Implements and assists with community vaccine and immunization clinics.
  • EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS
    • Minimum requirement:
      • Graduate from accredited nursing program (RN or LPN).
      • Maintains active RN or LPN licensure in Kentucky-multistate (or West Virginia-multistate) and Ohio
      • Five years of nursing experience.
      • Maintains American Heart Association Basic Life Support (BLS) certification.
      • Valid Driver's license.

  • WORKING ENVIRONMENT
    • Works indoors/outdoors in an office/clinic/mobile unit setting
    • May take equipment/supplies offsite to teach at remote locations
    • Required to travel to and from event locations
    • The noise level is usually quiet
  • PHYSICAL DEMANDS
    • Stands, sits, screens and teaches requested information throughout the day.
    • Frequently required stand and walk
    • Regularly required to sit; use hands to finger; handle or feel objects, tools or controls; reach with hands and arms; and talk or hear
    • Occasionally required to climb or balance and stoop, kneel, crouch or crawl.
    • Regularly required to operate a computer and AV equipment
    • Regularly required to lift and/or move up to 10 pounds, frequently lift and/or move 25 pounds and occasionally lift and/or move up to 50 pounds
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception and the ability to adjust focus
