JOB SUMMARY

Coordinates total nursing care for the patients at home, participates in patient and family teaching and provides leadership by working cooperatively with ancillary nursing and care team personnel in maintaining standards for professional nursing practice in the home.

DUTIES AND ESSENTIAL FUNCTIONS
-
Overall performance rating of below indicated essential job duties:

Home Health positions providing direct patient care in the patient home will maintain the following:
-
keep their assigned Home Health provided cell phone with them and turned on at all times during their scheduled shifts
-
keep their assigned Ever-Bridge phone application on and utilize in accordance to Home Health guidelines
-
keep their assigned Safety Connection badge charged and with them at all times during their scheduled shifts and will utilize Safety Connection badge in accordance to Home Health guidelines
-

Assesses the patient's condition, home environment, and nursing needs; sets goals, prescribes appropriate nursing actions to meet those goals and the physical, psychological, social, and rehabilitative needs of the patient, including discharge planning.

Initiates and/or updates a plan of treatment that is appropriate for attainment of specific and measurable goals for each patient based on the assessment of the patient's status and evaluation of the effectiveness of the orders.

Performs and documents prescribed treatments accordingly.
-
Utilizes organization-wide safe medication practices including, but not limited to the 5 rights of medication administration, barcode scanning and other medication safety processes specific to the unit/department. Knows actions and usual dosage, indications and side effects of medications administered.

Monitors and documents the patients’ response to treatments and education per regulatory guidelines.

Coordinates and delegates care in accordance with patient needs, capabilities and qualifications and collaborates with other members of the Home Health team and physicians to improve patient health and well-being.

Submits required documentation in accordance to regulator and agency time limits.

EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
-
Minimum requirement:
-
Graduate from an accredited school of nursing for Registered Nurses.
-
Maintains active RN license in the Commonwealth of Kentucky (or other eNLC state with multistate privileges)
-
Achieves and maintains American Heart Association Basic Life Support (BLS).
-
Minimum 2 years clinical nursing experience.
-
Current driver’s license and daily access to a reliable vehicle.
-

Preferred qualifications:

Bachelor’s Degree in Nursing
