JOB SUMMARY

To collect, maintain and make available to authorized user, timely, accurate and complete patient health information. Prioritizes, locates, electronically tracks and sees to the appropriate transport of records requested in accordance with established standards.

DUTIES AND ESSENTIAL FUNCTIONS

Overall performance rating of below indicated essential job duties:

Know the Epic system; Siemen’s imaging system; Dolbey transcription system; and Image Now scanning system. Understands the terminal digit filing system and clerical office systems.

Utilizes Review Station for receiving and sending of images to and from other hospitals. Utilizes computer information system (EPIC, PACS) in the department, with a multiple interfaces to various computer systems hospital wide, to obtain information which may directly or indirectly pertain to completing a customer request for information.

Know legal rights of patients for release of information, legal and regulatory guidelines and how to prepare invoices and appropriate correspondence for third party payers and attorneys. Maintains a high level of confidentiality and adheres to all HIPAA regulations when dealing with protected health information. Analyze and validate request for protected health information.

Performs quantitative and qualitative analysis on medical records according to standards established by the Medical Staff Quality and/or Medical Executive Committee, The Joint Commission and other licensing agencies.

Understand the deficiency process, the scanning process, how to deficiency a chart and follow-up on incomplete/delinquent records. Reviews medical records for completeness and assists physicians with documentation completion; following the medical record deficiency guidelines.

Determines appropriateness of communication and follow through with various hospital departments, internal and external physician offices, patients, etc. which may provide information related to locating films necessary for patient care related requests

Maintains active and inactive filing and storage systems for massive numbers of films, in multiple locations.

EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS

Minimum requirement:
-
High school diploma or general education degree (GED).
-
35 correct words per minute

Preferred qualifications:
-
One to three months related experience and/or training or medical record experience
