Job Description:
Job Summary
Responsible for providing comprehensive medical case management and social services to patients, ensuring a seamless continuum of care. Coordinates with multidisciplinary healthcare teams to address social needs, reduce barriers to care, and support medical care for vulnerable populations. Assesses psychosocial factors, formulates and implements discharge plans, and delivers follow-up services as needed. Collaborates with providers, ancillary staff, and community agencies to connect clients with essential resources. Facilitates patient engagement and supports improved health outcomes through dedicated case management and advocacy.
Essential Functions
• Serves as a member of the multidisciplinary health care team to provide patient services.
• Performs and documents psychosocial assessments for patients.
• Assists clients in accessing medical and social services through direct support or referrals.
• Empowers clients toward self-sufficiency and coordinates delivery of services to prevent duplication.
• Assists with patient care in clinical or home visit settings.
• Serves as liaison with internal departments and external agencies to ensure continuity of care.
• Provides patient education on disease transmission, medications, and healthy lifestyles.
• Assures accurate and timely care documentation in compliance with program guidelines.
• Seeks and assists with applications for financial assistance and expanded social services.
• Performs other duties as assigned.
Education Requirement
Bachelor’s degree in social work, or a related field
Master’s degree in social work is preferred
Experience Requirement
2-3 years of experience in social work, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
