Clinical Pharmacist - Infectious Diseases (Antimicrobial Stewardship)

  • JOB SUMMARY
    • Responsible for the provision of safe, appropriate, and cost-effective antimicrobial medication therapy across the services lines of KDHS
  • DUTIES AND ESSENTIAL FUNCTIONS
    • Overall performance rating of below indicated essential job duties:
      • Works collaboratively with other clinicians and health-care providers to implement and maintain innovative antimicrobial stewardship management programs
      • Participate in the education and development of medication us management programs including clinical guidelines, pathways, disease management and annual antibiograms
      • Plan, facilitate and co-lead the Antimicrobial Stewardship subcommittee of Pharmacy & Therapeutics Committee in collaboration with the Infectious Diseases physician leader
      • Represents pharmacy on various committees, staff meetings and professional meetings as needed as it relates to antimicrobial stewardship
      • Lead the ongoing review of antimicrobial medication utilization evaluations and coordinate efforts to meet drug budget targets regarding antimicrobial use
      • Maintains up-to-date knowledge of the profession through participation in organization memberships; attendance at meetings, lectures, in-service, seminars, conferences; reading journals; complying with National Standards of Continuing Education in order to maintain professional competency and proficiency
      • Participates in precepting experiential training for PharmD students and UK KDMC residents
  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
    • Minimum requirement:
      • Graduate of an ACPE accredited school of pharmacy with a BS Pharmacy for PharmD degree
      • Completion of an ASHP accredited PGY1 residency
      • Completion of Board Certification in Pharmacotherapy (BCPS), Infectious Diseases (BCIDP), and/or equivalent certificate program (MAD-ID Basic and Advanced, SIDP, etc.)
      • Active member of a professional organization with emphasis on infectious diseases (i.e. ASHP, ACCP, MAD-ID, SIDP, IDSA)
      • Kentucky Board of Pharmacy Pharmacist license
      • Ohio Pharmacist license obtained within 90 days of hire 
        • Achieves and maintains American Heart Association Basic Life Support (BLS) 
          • Achieves and maintains American Heart Association Advanced Cardiac Life Support within first 90 days of employment.

             

  • Preferred qualifications:
    • Completion of an ASHP accredited PGY2 Residency in Infectious Disease

    • Constantly works near moving mechanical parts
    • Constantly exposed to blood and/or body fluids and infection. 
    • Occasionally exposed to toxic or caustic chemicals
    • Occasionally exposed to risk of radiation
    • The noise level in the work environment is usually moderate.

    • Occasionally required to maintain in a stationary position (standing or sitting).
    • Frequently required to walk and move about.
    • Frequently required to operate a computer and other equipment.
    • Frequently required to use fine motor skills to handle and operate instruments/devices.

    • Frequently required to lift and/or move items up to 50 pounds.
    • Occasionally required to lift and/or move items up to 100 pounds.
    • Constantly required to communicate with patients and other care team requiring talking and hearing. 
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
    • Occasionally required to position self in order to meet patient needs.
