JOB SUMMARY

Specializes in diagnosis and treatment of speech, language and voice problems as related to human communication, as well as the diagnosis and treatment of dysphagia.

DUTIES AND ESSENTIAL FUNCTIONS
-
Overall performance rating of below indicated essential job duties:
-
Performs evaluation on patients referred in a timely manner and formulates diagnoses based on assessment findings.
-
Establishes individualized treatment plan and goals based on diagnostic testing.
-
Provides treatment for those communication areas found to have deficit during the evaluation.
-
Completes documentation according to practice setting guidelines.
-
Collaborates with other members of the healthcare team to identify needs and continued services once discharged from current level of care.

EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
-
Minimum requirement:
-
Master’s Degree in the field of Speech/Language Pathology
-
Maintains active ASHA Certificate of Clinical Competence (CCC) in Speech Language Pathology, or if in Clinical Fellowship Year (CFY), obtained when meets ASHA requirements for certification.
-
Current active Speech Pathology license in the State of Kentucky and other states as required. Interim license status accepted if in Clinical Fellowship Year.

Achieves and maintains American Heart Association Basic Life Support (BLS)
