Wellness Facilitator

  • JOB SUMMARY
    • Assists the department with clerical/administrative duties in an effort to maintain smooth operations of department, enhanced customer experience and improved quality.  
    • Exhibits enthusiasm, respect for boundaries, flexible operating hours, and demonstrates ability to work with diverse backgrounds
  • DUTIES AND ESSENTIAL FUNCTIONS
    • Overall performance rating of below indicated essential job duties:
    • Ensures that all general office duties such as, but not limited to mailing, word processing, filing/scanning and paperwork processing are done in a professional and efficient manner.
    • Orders supplies and maintains working order of equipment to support the team.
    • Courteously greets customers and answers phone calls and routes calls or takes accurate messages as appropriate and follows-up as necessary.
    • Assists customers with questions or concerns and provides appropriate answers or directs to appropriate individual.
    • Maintains organized work area for optimal efficiency.
    • Provides individualized health coaching to engage clients in appropriate services and resources and assist with coordination of care.
    • Promotes wellness services and engagement with primary care.
    • Maintains appropriate professional standards and provides appropriate follow-up
  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
    • Minimum requirement:
      • High School Diploma or GED.  
    • Preferred qualifications:
      • One year healthcare related experience
  • WORKING ENVIRONMENT
    • Works indoors in an office/clinic setting
    • The noise level is usually moderate

    • Constantly required to maintain a stationary position behind a computer.
    • Frequently required to move about inside the department.
    • Constantly required to communicate telephonically and face to face with colleagues and customers.
    • Constantly required to operate a computer and telephone.
    • Constantly required to lift and/or move up to 10 pounds.
    • Frequently required to lift and/or move up to 25 pounds.
    • Occasionally required to lift and/or move up to 50 pounds
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception
